Patient Information.

Our goal is to provide you with the most comprehensive, quality healthcare. We can help you coordinate all of your health care needs on-site. We hope that this guide will answer most of the questions that you may have. Our friendly and knowledgeable staff is available for further information.

Appointments
For your convenience; day, evening and weekend office hours are available by appointment. When scheduling an appointment, please inform the staff as to the nature and urgency of your problem. In the cases of emergency or problems that require special attention, every effort will be made to accommodate you as quickly as possible. In addition, a 24-hour answering service is available for your safety.If you need to re-schedule your appointment please contact our staff at least 24 hours in advance.

Insurance and Payments
We accept most major insurance plans, including HMO’s, PPO’s and Medicare. Be sure to bring your insurance ID card to your appointment. To facilitate your insurance claims, we submit an itemized bill directly to your insurance plan.

Some services must be paid out-of-pocket. A fee schedule is available upon request for those services. 

Fees for lab work are the responsibility of the patient.  Laboratory services are billed directly to the patient by a third-party.   Please call us regarding specific inquiries.

Co-Payments
All co-payments must be paid in full at the time of service. Co-payments vary based upon your insurance provider.

We accept cash, personal checks, and bank debit cards, American Express, Visa and MasterCard.

Referrals
In the event that additional services are needed, our staff will make the appropriate referrals to providers within our strong network of affiliations. We will also insure that you have the proper authorization from your insurance company for any necessary diagnostic testing or medical consultations. Referral requests must be made at least 72 hours in advance.

Test Results
If you have laboratory tests done at BWS, please speak to your physician and medical assistant regarding notification of your results. Most tests take 7 business days for the results. If you have abnormal test results you will receive a phone call from your doctor within 2-3 days after the results are received by our office. If your test results are normal, depending on the test, you should expect a card in the mail within 2 –3 weeks.

Change of Patient Information
It is important that we have your most current information on file. If you change your address, telephone number or insurance provider please be sure to provide your new information to our staff as soon as possible.

Parking
Convenient discount parking is available at Stable Car Parking located at 9201 Fourth Avenue. Entrance to parking garage is on Fourth Avenue between 92nd and 93rd Streets. Ask the front desk to validate your ticket.
